Abnormal Uterine Bleeding

Basics

Abnormal Uterine Bleeding (AUB) is defined as any uterine bleeding that does not fit the pattern of normal menstrual periods. It can occur before menarche or after menopause. Terms used to describe patterns of Abnormal Uterine Bleeding, based on periodicity and quantity of flow, are:

Menorrhagia Bleeding excessive in both amount and duration of flow occurring at regular intervals. 

Metrorrhagia Bleeding occurring at irregular intervals and in varying amounts but not excessive.

Menometrorrhagia Bleeding, usually excessive and prolonged, occurring at frequent and irregular intervals.

Polymenorrhea Frequent but regular episodes of bleeding, usually with intervals of 21 days or less.

Oligomenorrhea Infrequent, irregular episodes of bleeding usually at intervals greater than 35 days.

Amenorrhea No uterine bleeding for at least 6 months.

Intermenstrual Uterine bleeding between regular cycles.

Symptoms

Abnormal Uterine Bleeding (AUB) may be heavier flow, more frequent bleeding, unscheduled bleeding , or bleeding of any kind in a post-menopausal patient not on hormone replacement therapy.
Headaches often occur in AUB due to increased instability of the blood vessels. Other common complaints of AUB include cold hands and feet, forgetfulness and inability to concentrate.

Prevention

IUDs should be used with extreme caution as they can cause abnormal bleeding . Other than this there are no precautionary methods as such for this condition.

Causes

Abnormal Uterine Bleeding can occur at any age. A common cause of abnormal bleeding in young women and teenagers is pregnancy. Although usually a woman will stop menstruating if she is pregnant, bleeding during pregnancy is not uncommon.

Women who experience bleeding in the first few months of a normal pregnancy may have too much estrogen, progesterone polyps or fibroids. If the pregnancy is abnormal, such as a tubal pregnancy or impending miscarriage, the bleeding may be interpreted as being from a menstrual period rather than a complication of pregnancy.

Other causes include a thickened endometrium and uterine Long standing problems. Uterine Long standing problems is more common in older women than in younger women. But Long standing problems is not always the cause of Abnormal Uterine Bleeding.

In the years before menopause, there are months when ovulation does not take place, which might result in Abnormal Uterine Bleeding. Thickening of the lining of the uterus is another cause of bleeding in women in their 40s. This thickening can be a warning of uterine Long standing problems.

Menorrhagia (excessive uterine bleeding) most commonly results from a hormonal imbalance in adolescents in the years following onset of menstruation and in women who are approaching menopause.

Birth control pills or the Norplant birth control device can also cause abnormal bleeding. If an egg isn't released during the menstrual cycle, abnormal bleeding either light spotting between periods or heavy bleeding during the period, may be experienced. Women using intrauterine devices for birth control may also experience excessive or prolonged periods.

Diagnosis

The diagnostic approach to Abnormal Uterine Bleeding needs not be intimidating. Clinical information, along with the reproductive stage of the patient, narrows the etiologic possibilities. The etiology of abnormal bleeding can be uncovered in a cost-effective way if the physician knows the most common causes of bleeding in each age group and uses a logical diagnostic approach.
